| The Day Habilitation (Day Hab) program provides services and supports to assist individuals in achieving their maximum level of independence in daily living. The program has several sites, the main one being Sullivan Day Hab at 235 North Avenue. Programs operate Monday through Friday and a person may attend either full or part time. Transportation is provided to and from the programs by the Yates ARC Transportation Department. Day Hab programs provide training and assistance to enhance the person's independence and productivity in activities of daily living, communication, social skills, mobility, and community inclusion. Nursing services are available. The Sullivan Day Hab site also offers services in psychology, occupational and physical therapy. Each person is directly involved in developing a program plan based on their particular interests and needs. Family members and others who know the person well are encouraged to participate, with trained staff, in this process. Day Hab has the flexibility to meet a variety of individual needs and interests as well as promoting increased community involvement. People are offered daily choices of recreational and social activities to enhance their skills. Volunteer opportunities are also available to foster mutually rewarding relationships within the local community. Who is Eligible to Participate: Any individual who has a developmental disability, is over the age of 18 and is eligible for the Medicaid Waiver is welcome in Day Hab. |  | Phil: "I like to keep busy shredding, and I also like my new walker." | | | | Through our OT services Phil recently received a walker better suited to meet his particular needs.
